As described on NPR regarding this report on the now clear benefits of coffee consumption, early (and not very well done) studies linking coffee drinking with increased mortality and morbidity neglected to disentangle coffee drinking from smoking.
Turns out that when these early studies were done people who drank coffee and smoked outnumbered those who drank coffee and did not smoke. That is, those who didn't drink coffee also tended not to smoke (probably Mormons.)
In any case, at the time, smoking was not recognized as harmful so smoking behavior was not controlled for in the early health effects studies of coffee consumption.
